Writing duties in the preparation of the book were distributed equally between the authors. The writing model adopted was each author took primary responsibility for the background research and drafting of certain chapters, which were then reviewed and revised by the partner author, before a final draft was prepared together. The final discussion chapter was then co-written by both authors.
