This paper provides full technical details of the implementation of a novel idea which was recognised through selection within a highly competitive process as a "post-deadline" oral presentation at the leading conference in the field, OFC 2008 (San Diego). Such papers are deemed to be of such importance they warrant immediate dissemination. This advance on coding attracted immediate industrial interest with Oki sending a delegation from Japan (G, C Gupta, ID_in_audit_file@oki.com. Further funding has been provided by Avago (GBP467k, Dr David Cunningham, ID_in_audit_file@avagotech.com) to develop the work. The work also resulted in an invited paper at OFC 2011 (Los Angeles).
