Each author took individual responsibility for one section of this volume. Moore was solely responsible for Part III (writing Chapter 8 and editing chapters 9-11). The authors worked on the book’s general introduction together. As Denison undertook the bulk of the adminstrative duties for the volume (liaising with the publisher, overseeing deadlines etc), the editorial team decided that his name should go first on the volume. All other names are alphabetised to reflect the shared workload.
