I was the sole individual who conceived the study and then Nick Jeffery and I designed it together and drove the work. It was a multi-centre study so the other authors contributed to the conduct of the study at their centres, but it was Greenhalgh, Jeffery and Watson who played the predominant roles in organisation, conduct, analysis and interpretation of result and in drafting the output.
