In 2007, NICE issued guidelines that all patients in UK hospitals should have their observations recorded and graded in a system called “track-and-trigger”. This paper describes the design of our evidence-based track-and-trigger system, which is now the system used in all hospital wards of the Oxford University Hospitals NHS Trust (thousands of patients observed each day). The research described in the paper has also resulted in Prof. Tarassenko being asked to give an invited lecture at the first Science conference of the Royal College of Physicians in November 2013.
