This book featured papers presented at a conference that Middleton co-conceived and organized with Billy G Smith and which was held in Montana in 2003. With Smith, he set the intellectual agenda for the conference, solicited and edited the chapters, co-wrote the introduction (pp. 1-16), and contributed his own chapter, "A Class Struggle in New York?" pp. 88-99. The two editors shared the editorial work equally and the press had the proposal and book manuscript peer reviewed.
