Based on an invited paper at the annual conference of the Innocence Network held in Cincinnatti, this article appears in a Special Issue on: 'An International Exploration of Wrongful Convictions.' It critically examines three institutions of the England and Wales criminal justice system, bringing them together for the first time to assess their 'success' in addressing miscarriages of justice. This includes the first academic analysis of the role of the Forensic Regulator. It is included in the first special issue taking a truly international perspective of wrongful convictions with authors from Asia, the US, South America, Africa, Australia and Europe.
