Medicine serves as an archetype for self-regulation but varying standards and issues of patient safety have led to reform. This chapter compares the policy response in France with the English NHS. An analysis of policy instruments shows similarities and differences. Both countries have mechanisms for evidence-based practice and peer-based reaccreditation but cost reduction is achieved by different means: administrative reforms in France and an internal market in the NHS. In France, self-governance remains. In the UK, there has been a shift to partnership governance. We explain the factors determining the pathways to reform. The research contributed to collaborative scholarship.
