This monograph is the product of eight years of research and writing, and was my primary research focus during this time. It is based on research that I started in 2003 as a PhD student. After completing my PhD in 2007 and writing several articles based upon it, I conducted additional research from 2009-2010 for the book. I took sole responsibility for all aspects of the research, which involved extensive periods of time conducting interviews, conducting participant observation and analysing documents.
