This paper is a journal version of a paper at the 2009 Computability in Europe conference, held in Heidelberg, Germany. The paper was one of only two other papers at the conference recommended to the Annals of Pure and Applied Logic by the program committee. This paper was a core piece of Dr Roy's dissertation, which was awarded a George M. Sprowls Award in 2011 by the MIT Electrical Engineering and Computer Science department, given to the top Computer Science dissertations in each year.
