<28> The NHS is faced with inefficient patient discharge, frequent readmissions, long waiting lists and bed blocking. Effective Discharge Planning (DP) requires practitioners to have knowledge to make informed decisions about patients discharge. This paper examines the role of Knowledge Management (KM) in sharing and using tacit knowledge to create patient discharge pathways. It details the factors resulting in inadequate DP, and demonstrates the use of Internet of Things (IoT) and Machine2Machine (M2M) as candidate technologies or solutions which can help reduce the problem. The use of IoT-linked devices can support practitioners in making decisions about patient discharge.
