This research, supported by National Science Council (Taiwan) and the Aviation Safety Council, undertaken with the National Defense University (Taipei) extends a technique developed by the authors providing the probabilities for co-occurrence of categories across adjacent levels of the Human Factors Analysis and Classification System. For the first time empirical evidence has been obtained supporting Reason’s model of human error having its root causes in managerial failures. Further development of this work has been used by Taiwanese Air Force and National airlines as the basis for developing safety training.
