Additional information

Forcer had the lead role at all stages, including preparation and submission of the proposal to the Editorial Board, commissioning of contributors, sole writing of the Introduction (5,000 words), and acquisition of the cover image. The main analysis presented in Forcer and Wagstaff's co-authored article (6,500 words) was prepared jointly and equally by the authors; after discussion with Wagstaff Forcer then drafted the introduction and conclusion to the co-authored article, which was reviewed by Wagstaff. Forcer took on slightly more (60:40) of the copy editing and proofing of the complete volume.
