<25>The approach developed in this paper to model flow through patient pathways, including phases in hospital, social services and community care, is based on stroke patients’ data from the Belfast City Hospital, and is being used currently to inform policy and planning for stroke patients in the Belfast Health and Social Care Trust to derive efficiency savings (reference: ken.fullerton@belfasttrust.hscni.net). The approach was developed as part of the EPSRC-funded RIGHT (EP/E019900/1) and MATCH (EP/F063822/1) projects and forms part of an Impact Case Study in this submission.
