The volume was entirely planned by the editor. Each chapter was individually commissioned, and a meeting was held early in the project to establish overlap and connections. Chapters were substantially edited and the volume was prepared for CUP by the editor (with a research assistant in first instance). In some instances, the editor did additional picture research for the contributors. He was also sole author of the introduction and of three of the chapters.
