It took me many years to put this book together. I have been teaching and rehearsing this material in undergraduate and postgraduate teaching for about fifteen years. Formally, some of it had its first outing at conferences in 2003 and 2004 and in subsequent years I have shared more of its content in conference and symposia form on at least an annual basis. Over the years, a number of published essays and journal articles have appeared based on material that was refigured in this book. None of this previously published material is being submitted separately as REF outputs.
