a. The author made a substantial contribution to the conception and design of the study; the organisation of the conduct of the study; carrying out the study (including acquisition of study data); the analysis and interpretation of study data and b. The author helped draft the output. (Health Technology Assessment (HTA) reports are viewed as important pieces of collaborative research that make a valuable contribution to the evidence base informing care. Within our report there were two distinct but equally important substantial components making such a contribution: a systematic review and a modelling of cost effectiveness (using the results from the review). While both authors contributed to both aspects, it was Beverley French that led the systematic review component (synthesis and write-up) and Michael Leathley that led the cost-effectiveness modelling (analysis and write-up).)
