This book is the fruit of research done over the last 10 years, starting with my doctoral dissertation (2005). Chapters 2–5 are substantially reworked chapters of the dissertation. The scope of the book is much greater than the dissertation’s and involved substantial additional research. The remaining eight chapters (including prologue and epilogue) mostly have their origin in separate papers given at conferences and workshops during my research fellowship in France (2010–11). The quantity of work involved therefore clearly exceeds, several times, that required to produce two single-weighted outputs; this work has displaced opportunities to produce other comparable outputs.
