This essay was written in its entirety by Kennedy, with comments from his two collaborators, Miller in the USA (University of Columbia-Missouri) and Gurrin in the Republic of Ireland (NUI Maynooth). The editors and referees of this volume also made useful critical comments. The identification of the case study and its associated archival materials was undertaken by Kennedy. His other contributions included much of the analysis, in particular the theoretical and statistical work, so his overall contribution was in the region of 80%.
