This paper is the result of an industrial collaboration with Element 6 Ltd [Principal Scientist: ian.friel@e6.com]. This research was also the subject of an invited talk in September 2010 at the main UK conference in optics, namely Photon 10, sponsored by the UK Institute of Physics.  This paper provided the foundation for the field of diamond photonics and was used as a proof-of-concept demonstration for the European Research Council Starting Grant awarded to Strathclyde in 2011 [Diamond Lasers: Revolutionising Laser Engineering, ERC-2011, PE7].
