As first author in the team I made a substantial contribution to: the conception and design of the study, the organisation of the conduct of the study - including identifying the co-authors, developing the guidelines and breadth of the review, carrying out the study including assisting with the critique of articles, interpretation of study data - in particular leading the process of gaining consensus on all of the Grade criteria rankings and associated recommendations. As author I was primarily responsible for drafting the output, sought consultation and agreement from the rest of the team for the final version of the manuscript.
